Grant document checklist

Proof of income — choose ONE path

• Path A (simplest): a full award letter from the last 12 months for HEAP, SNAP (food stamps), TANF or SSI. Then no other income paperwork is needed.
• Path B: income documents for every adult (18+) in the household —

• Recent pay stubs (most recent month)
• Social Security or pension award letters
• Unemployment or disability benefit statements
• Most recent tax return if self-employed
• Child support or alimony received

About the home

• A recent utility bill (electric and/or heating) in the name of someone in the household
• If you heat with oil or propane: a recent delivery receipt or statement

If you rent

• Landlord’s name and phone number
• Landlord’s signed approval for the work (we provide the form)

For the application

• Names of everyone living in the home
• A phone number and email where the grant program can reach you
